I am really confused about how the KY Smallmouth license plates are working. You make a $25 donation and that counts as a "Pre-order" for the plate....but when do you actually get the plate? The transportation department doesn't even show the Smallmouth plate as an option on their website....I'm confused.
Its going to be at least another year wait. Here's what my last email about it said.

Thank you for ordering the Kentucky Fish and Wildlife Foundation’s Smallmouth license plate. We are very sorry that this process is taking so much longer than expected. The Kentucky Department of Transportation is experiencing some issues implementing their new KAVIS system to each of the 120 county courthouses. We continue to ask for updates and below is the response that we recently received. Unfortunately it looks like it is still going to be a while. Thanks again for your purchase and for your support of youth conservation efforts here in Kentucky. We will send you and update as soon as new information is received. This plate is going to be the best looking plate on the road when it finally arrives.

That is great news! Plate manufacturing, beginning in late 2014/early 2015 after KAVIS is implemented, will be done in the order they are received. Your organization will be the first new plate manufactured within our new KAVIS system! Unfortunately, I cannot give you a specific date.
However, once we are ready to begin the process, programming changes, plate sample creation and distribution for the organization and the Cabinet to give final approval (for color saturations) and notification postcard creation (for your 900+ applicants), you will be a part of weekly status notifications. As these activities positively progress, a date will be finalized. Once confirmed, this will be the time for the organization to think about advertising the release. The Cabinet notifies the public through our MVL website and through internal communications to all 120 county clerk offices.
