i think there are several issues.

generally speaking 4-5" bait -> 4/0 or 5/0 hook
treble hooks can help but are not fail proof and defeat the weedless rigging option
consider swimbait hook sets as you do topwater -> ignore the initial attack and wait for the rod to load up (get heavy) before setting the hook. all too many times i have found that the fish seem to initially nip the tail before engulfing the lure. i have much better hookup rates by waiting for the heavy feeling. i must also say that i have yet to gut hook a fish on a swimbait with this "delayed" hook set.
if fishing open water - consider a jig head with an exposed head as opposed to a swim bait hook with lead on the shank.
lastly, l do not like the money minnow design. not knocking your bait but the extra "depth" at the head of the bait was less than ideal for me with regard to the weighted swim bait hooks. too much plastic to pull through for a hookset and i did not like the profile after rigging it. i prefer the more torpedo shaped swim baits.
