My apologies for not getting a reply. Our member who takes care of that has had some difficulty with ill family members. Please e-mail me here privately with your address and I will send you a form personally. Also, if you would like a Kid and Co. registration form I will include that also. We play hostess to about 120 kids and their parents for a family day of fun and prizes. Again, sorry about the delay. Please e-mail me privately and I will send you a form.
